2. I hereby acknowledge the authority of Warlord Hockey Academy and agree to carry out and abide by their constitutions, bylaws, rules and regulations.

3. I understand that submission of this application does not guarantee a position. I will make myself available for an interview with the Coach Selection Committee, given reasonable notice, should an interview be required. I further understand that the association and its committee have the final responsibility of selecting applicants and I have no resource should I not be selected.

4. By way of this application, I give permission to the Warlord Hockey Academy governing body, the AAU, to pursue a criminal record search of myself should I be selected for a position.
